Palm Shores Behavioral Health Center accepts referrals from parents and family members, professionals, schools, physicians and providers from the community, both in state and out of state.

Admissions are conducted Monday through Friday, but special arrangements can be made for weekends if necessary, on a case by case basis.

When making a referral the following information facilitates the admission process:

• Psychiatric evaluation (within the past year)
• Presenting problems and history of treatment
• Clinical records
• Medical and immunization records
• Psychological history
• Medical, medication and dental history
• School records
• Insurance information

The Director of Admissions will review all clinical information with Palm Shores Behavioral Health Center Clinical Treatment Team for approval and admission.
