What to Bring to Palm Shores Behavioral Health
Please discuss items prior to admission and arrival with the Admissions Department.
- Seven changes of clothes (must be string free – no drawstrings)
- Seven changes of undergarments (bras may not have underwire)
- Tennis shoes (no shoestrings, ties will be provided for safety by Palm Shores Behavioral Health Center)
- Immunizations
- Custody/legal documents as applicable
- Copy of photo identification for parents/guardians
- Proof of residency to register for school (copy of lease/rental agreement, deed, or electric bill)
- Insurance card
